Application

Thank you for your interest in applying to Fremont Middle School. 

Please plan to assist us with enrollment/registration by providing us with:

1. Original state certified birth certificate.
2. Two (2) proofs of your residency
3. Updated Immunization record
4. Enrollment packet filled out completely.

Enrollment packets may be picked up at the Middle School office during the school year and the Administration office during the summer.

If you live outside of Fremont School District, please call the Administration office at 231-924-2350.
